What ABDM integration actually means
The Ayushman Bharat Digital Mission (ABDM) connects patients, healthcare providers and health-data users through a set of shared registries, a gateway and consent-based record exchange. For a hospital, “integrating with ABDM” means your HIS, EMR or clinic software can do four things: create and verify patient ABHA numbers, link a patient's visits to their ABHA, share health records when the patient consents, and, if you need it, request records from other providers.
Nothing here replaces your existing software. It is an integration layer that talks to the ABDM gateway on one side and your systems on the other.
The building blocks
- ✓ABHA number and ABHA address — a patient's health account identifier and a human-readable address. See how ABHA creation and linking works in a HIS.
- ✓Health Facility Registry (HFR) and Healthcare Professionals Registry (HPR) — registries that identify your facility and your clinicians on the network.
- ✓Health Information Provider (HIP) — a facility that holds records and shares them with consent. Health Information User (HIU) — a party that requests records with consent. Read HIP vs HIU: which does your hospital need.
- ✓Consent manager — the patient-facing component that records approvals and revocations.
- ✓ABDM gateway — the routing layer for discovery, linking, consent notifications and data transfer.
- ✓FHIR-based health records — records exchanged as FHIR R4 bundles following Indian profiles. If FHIR is new to you, start with FHIR vs HL7 v2 for Indian hospitals.
The patient journey, step by step
- 1.At registration the patient shares or creates an ABHA, for example by scanning a QR code at the front desk (often called Scan & Share).
- 2.The HIS stores the ABHA against the patient record and verifies it.
- 3.After a visit, the hospital links the visit — a “care context” — to the patient's ABHA so the patient can see it in their health app.
- 4.When another provider or the patient asks for records, the consent manager asks the patient to approve.
- 5.On approval, the HIP prepares the records as FHIR bundles, encrypts them using the method ABDM specifies, and sends them through the gateway.
Milestones and testing
ABDM groups integration into milestones: M1 for ABHA creation and verification, M2 for acting as a HIP, and M3 for acting as a HIU. Each is tested in the ABDM sandbox before production access. Our M1, M2 and M3 checklist shows what to prepare for each.
Always check the current National Health Authority documentation: specifications and onboarding steps are updated periodically.
Where hospitals get stuck
- ✓Patient identity: the same patient registered twice with different spellings, so records link to the wrong person.
- ✓Data quality: free-text notes and scanned PDFs that have to be turned into structured FHIR records.
- ✓Workflow: front-desk staff who are asked to add steps without a visible benefit.
- ✓Privacy: consent screens and audit trails that were not planned for. See our DPDP checklist for hospitals.
How to start
Begin with a short discovery: list the systems that hold patient data, decide whether you need to be a HIP, an HIU or both, and pick one department for a pilot. A scoped pilot teaches you more than a hospital-wide plan on paper. If you would like help, talk to our team.
Frequently asked questions
Is ABDM integration mandatory for hospitals?
Participation is voluntary in general, but some schemes and programmes link benefits or empanelment to ABDM participation. Check the current requirements that apply to your facility and state.
Do we need to replace our HIS to integrate with ABDM?
Usually not. The integration is added alongside your existing HIS or EMR, either by the vendor or by a third party that connects to your system's APIs or database.
What standard does ABDM use for health records?
Health records are exchanged as FHIR R4 bundles following the profiles published for India.
How long does ABDM integration take?
It depends on how open your HIS is, the quality of your patient data and how many milestones you target. A written plan after a short discovery gives a realistic estimate for your case.
